To qualify in 2025, you must be a West Virginia resident and at least 18 years old to apply as an adult patient. Applicants under 18 may qualify if a parent or legal guardian registers on the minor’s behalf and serves as the caregiver.
A diagnosis of a qualifying medical condition from the 2025 West Virginia list is required. Examples include cancer, epilepsy, multiple sclerosis, Parkinson’s disease, post-traumatic stress disorder (PTSD), and severe chronic or intractable pain, among others.
